STUDENT SUPPORT FORM

You Are Not Alone ❤️🤍💙

Everyone struggles sometimes and it’s okay to ask for help. Reaching out is a sign of strength, not weakness. We are here for you, and you don’t have to go through things alone.

⚠️ Important: This is not a crisis hotline or a substitute for emergency mental health services.

If you are in immediate danger or having a medical or mental health emergency, call 911 right away.

If you are feeling suicidal, please call 988 (Suicide & Crisis Lifeline) or text “HOME” to 741741 to connect with the Crisis Text Line (available 24/7).

For Students:

If you need support and it’s not an emergency, please fill out the referral form below. A staff member will check in with you by the next school day.

If you’re struggling with thoughts of suicide, please use 988 or the Crisis Text Line right away. If you do reach out to those hotlines, let us know by filling out the form with your name and number so we can follow up and support you.

Remember—you don’t have to keep things bottled up. Talk to someone you trust: your parent, teacher, counselor, school social worker, psychologist, or principal. Your well-being is our top priority.

For Parents:

If you are concerned about your child’s mental health, please fill out the referral form below so we can connect your child with support.

In an emergency, call 911 or take your child to the nearest hospital. Please also fill out the form so we are aware and can follow up.

All referrals are confidential and go directly to our site’s mental health team.
